Chronic Health Conditions
If your student(s) has been diagnosed with a chronic health condition:
All medical statements and medical/nursing forms must be completed ANNUALLY by the parent/guardian and/or the student's healthcare provider.
These forms are available in the nurse's office at your student's school. Completed forms and all supplies and medications should be brought to your student's health office.
If your student(s) has a life threatening food allergy, please complete the form available in your student's health office.
North Canyon High supports fostering nutritious eating habits so our district's students will be prepared for a healthier lifestyle.
Menus are analyzed annually to ensure that nutrient standards established by the U.S. Department of Agriculture are met.
The National School Lunch Program provides students who meet certain criteria free or reduced-price meals. Applications are available in our school cafeteria.
